No, Gai is most likely going to die. This has been built up since the Chuunin Exams and Kishi has killed other people of similar importance recently. If he doesn't die from this, I think he knows that everyone will rage hard. Well, except maybe Japan.

No, I'm honestly quite shocked he didn't come back with everyone else when they revived. Don't count him out yet, they'll find something.

Does he plan ahead before making new chapters or read fans assumptions online and base his decisions on that?

No, Kishimoto writes his story on his own with input from his assistants and editors. Kishimoto does not care about Guy a ton, he has also probably been told by editors or assistants that Guy is not a super popular character (Which he's not in Japan) so he will kill him off. Also Naruto has too many characters and can stand to kill a few, Guy is one of my favorite characters and his whole team is expendable no matter how much I don't want to admit that. Guy's team rocks, but if you thought any of the central Konoha teams would die, it would always be them.

Guy and Neji exist for the sake of two other characters (Lee and Hinata respectively), Lee is the only one who would realistically survive because he's meant to carry on Guy's will, and TenTen'a just there. If you are a writer and have to kill characters you logical kill the characters whose death's will have meaning and not pointless side characters who no one will react to.

Sai doesn't really have a real name since he was born and raised in ROOT iirc. Yamato's real name might be Tenzo, which is what Kakashi called him before when he was in ANBU.
